Heart rates of the Apollo 14 crew during multiple docking attempts It took 6 attempts to achieve a successful docking of the Command and Service Module (CSM) and the Lunar Module (LM) during Apollo 14. It was later determined that a small piece of debris likely prevented the mechanical system from automatically triggering.
